ABSTRACT

Objetivo: La leucoplasia oral es el desorden maligno de la mucosa bucal más prevalente a nivel global y su manejo clínico sigue siendo un desafío. Se llevó a cabo una revisión sistemática para determinar la eficacia clínica de la terapia fotodinámica mediada por ácido 5-aminolevulínico tópico como una alternativa de quimio-prevención para las diferen- tes formas clínicas de la leucoplasia oral. Materiales y métodos: Empleando términos MeSH, se realizó una búsqueda exhaustiva en diferentes bases digi- tales de ensayos clínicos publicados en inglés en los últimos 30 años acerca del uso de la terapia fotodinámica mediada por ácido 5-aminolevulínico tópico como fotosensibilizador, y radiación láser de baja intensidad o luz LED como posibles fuentes de iluminación. Resultados: La revisión sistematizada que aplicó la guía PRISMA mostró una eficacia del 88,6% para este modo de fototerapia en el manejo de leucoplasias orales, con un 60,7% de respuesta completa y 27,9% de respuesta parcial. Además, el tamaño de efecto fue mayor para las formas clíni- cas homogéneas con cambios displásicos, independientemen- te del tipo de fuente de luz. La ausencia de respuesta fue del 11,4%, pero la evidencia empleada en este análisis fue mo- derada. Conclusión: La terapia fotodinámica mediada por áci- do 5-aminolevulínico tópico parece ser una alternativa útil en el manejo onco-preventivo de lesiones de leucoplasia oral. Sin embargo, es recomendable ejecutar ensayos clínicos controla- dos y aleatorizados con metodologías homogéneas que per- mitan generar un meta-análisis con un alto nivel de evidencia


Aim: Oral leukoplakia is globally the most prevalent ma- lignant disorder of the oral mucosa and its clinical manage- ment remains a challenge. A systematic review was carried out to determine the clinical efficacy of photodynamic therapy mediated by topical 5-aminolevulinic acid as an alternative for chemoprevention in the different clinical forms of oral leu- koplakia. Materials and methods: Using MeSH terms, an ex- haustive search was carried out in different digital databases of clinical trials published in English in the last 30 years on the use of photodynamic therapy mediated by topical 5-ami- nolevulinic acid as a photosensitizer, and low-intensity laser radiation or LED light as possible lighting sources. Results: The systematized review using PRISMA guide- lines showed an efficacy of 88.6% for this mode of photother- apy in the management of oral leukoplakias, based on 60.7% of complete response and 27.9% of partial response. In addi- tion, the effect size was larger in homogeneous clinical forms with dysplastic changes, regardless of the type of light source. There was an 11.4% of absence of response, but the evidence used in this analysis was moderate. Conclusion: Photodynamic therapy mediated by topical 5-aminolevulinic acid seems to be a useful alternative in the onco-preventive management of oral leukoplakia lesions. However, it is recommendable to perform controlled and ran- domized clinical trials with homogeneous methodologies that allow the generation of a meta-analysis with a high level of evidence (AU)

ABSTRACT

OBJECTIVE@#To study the binding target of photosensitizer and bacteria in antimicrobial photodynamic therapy with computer-simulated target prediction and molecular docking research methods and to calculate the binding energy.@*METHODS@#The protein names of Porphyromonas gingivalis (Pg) were obtained and summarized in Uniprot database and RCSB PDB database; the structure diagrams of methy-lene blue were screened in SciFinder database, PubChem database, ChemSpider database, and Chemical Book, and ChemBioDraw software was used to draw and confirm the three-dimensional structure for target prediction and Cytoscape software was used to build a visual network diagram; a protein interaction network was searched and built between the methylene blue target and the common target of Pg in the String database; then we selected FimA, Mfa4, RgpB, and Kgp K1 proteins, used AutoDock software to calculate the docking energy of methylene blue and the above-mentioned proteins and performed molecular docking.@*RESULTS@#The target prediction results showed that there were 19 common targets between the 268 potential targets of methylene blue and 1 865 Pg proteins. The 19 targets were: groS, radA, rplA, dps, fabH, pyrG, thyA, panC, RHO, frdA, ileS, bioA, def, ddl, TPR, murA, lepB, cobT, and gyrB. The results of the molecular docking showed that methylene blue could bind to 9 sites of FimA protein, with a binding energy of -6.26 kcal/mol; with 4 sites of Mfa4 protein and hydrogen bond formation site GLU47, and the binding energy of -5.91 kcal/mol, the binding energy of LYS80, the hydrogen bond forming site of RgpB protein, was -5.14 kcal/mol, and the binding energy of 6 sites of Kgp K1 protein and the hydrogen bond forming site GLY1114 of -5.07 kcal/mol.@*CONCLUSION@#Computer simulation of target prediction and molecular docking technology can initially reveal the binding, degree of binding and binding sites of methylene blue and Pg proteins. This method provides a reference for future research on the screening of binding sites of photosensitizers to cells and bacteria.

ABSTRACT

Aim: To compare Enterococcus faecalis reduction after antimicrobial photodynamic therapy (aPDT) used with methylene blue, toluidine blue, tannin, and curcumin as photosensitizers, an adjunct to endodontic chemomechanical preparation (CMP) in root canals of human teeth. Methods: A total of 120 single-rooted teeth were divided into 6 groups (n = 20): G1- CMP and 2.5% sodium hypochlorite (NaOCl); G2- CMP and saline solution; G3- CMP, 2.5% NaOCl, and aPDT with 0.005% methylene blue; G4- CMP, 2.5% NaOCl, and aPDT with 0.005% toluidine blue; G5- CMP, 2.5% NaOCl, and aPDT with 0.005% tannin; and G6- CMP, 2.5% NaOCl, and aPDT with 0.005% curcumin. A portable semiconductor laser was used (660 nm, 100 mW, 1.8 J, 180s) in groups 1, 2, 3, 4, 5, and a blue LED light-curing (420-480 nm, 1200 mV/cm2 ) in G6. For all groups, a 5 min pre-irradiation time was applied. Samples were collected before (initial collection), immediately after (intermediate collection) and 7 days after CMP (final collection) for colony-forming unit (CFU) counting. The Kruskal-Wallis test and analysis of variance (ANOVA) were performed (p < 0.05; 95% confidence interval). Results: In between-group comparisons, there was no significant difference observed in the number of CFUs at the initial (p >< 0.001) and final collections (p >< 0.001) for G2 and G3. In within-group comparisons, the number of CFUs showed a decreasing trend in G4 (p = 0.007) and G5 (p = 0.001). Conclusion: Photosensitizers promoted E. faecalis reduction, with better results for tannin and curcumin. Alternative photosensitizers should be the focus of further studies.>< 0.05; 95% confidence interval). ABSTRACT Purpose: Keratoconus presents certain specificities in pediatric patients compared with adults. The greatest challenge is because the disease is typically more severe and progresses faster in children. This retrospective study aimed to report crosslinking procedure in patients under 18 years of age and their follow-up for at least 24 months after the procedure. Methods: Overall, 12 eyes from 10 patients were studied and data, such as visual acuity with and without correction, maximum keratometry, corneal thickness, foveal thickness, and endothelial microscopy, were assessed at both preoperative and postoperative visits. Corneal crosslinking was performed in all patients. Results: A tendency toward reduced Kmax and improved Corrected Distance Visual Acuity at all postoperative moments. Only one of the 12 eyes exhibited increased Kmax of more than 1 D during a time frame longer than 12 months. Regarding pachymetry, a tendency for corneal thinning was observed in the first four months after surgery. Conclusion: Encouraging results were obtained regarding the stabilization of the disease, progression, and procedural safety, corroborating to other authors' findings. The significance of early diagnosis and short-term follow-up were highlighted.


RESUMO Objetivo: O ceratocone na população pediátrica apresenta algumas particularidades em relação à população adulta. O maior desafio é devido à doença ser geralmente mais severa e rapidamente progressiva em crianças. Métodos: Este artigo utiliza uma análise retrospectiva para relatar o uso do crosslinking em jovens menores de 18 anos e sua evolução pelo menos 24 meses após o procedimento. Foram estudados 12 olhos de 10 pacientes, e dados como acuidade visual com e sem correção, ceratometria máxima, espessura corneana, espessura foveal e microscopia endotelial avaliados no pré e pós-operatórios. O crosslinking corneano foi realizado em todos os pacientes pelo mesmo cirurgião. Resultados: Observou-se uma tendência de redução do valor do Kmax e melhora da acuidade visual corrigida em todos os momentos de pós operatório. Com relação à paquimetria, observou-se afinamento corneano do ponto mais fino, nos primeiros quatro meses de pós-operatório. Conclusão: Resultados encorajadores foram obtidos com relação à estabilização da doença, progressão e segurança do procedimento, corroborando com as conclusões de outros autores. A importância do diagnóstico precoce e do acompanhamento a curto prazo do paciente deve ser destacada.

ABSTRACT

A terapia fotodinâmica antimicrobiana (TFDa) tem sido considerada uma alternativa para o tratamento de infecções cutâneas causadas por Acinetobacter baumannii. Entretanto, é necessária a busca por fotossensibilizadores ou compostos que potencializem seus efeitos. Os objetivos desse estudo foram testar uma nova Clorina e-6 (Fotoenticine FTC) como fotossensibilizador para TFDa sobre A. baumannii, e o Farnesol como potencializador dessa terapia. Além disso, os efeitos do FTC foram comparados ao Azul de Metileno (AM), um fotossensibilizador já aprovado para uso clínico. Para isso, foram empregados os seguintes métodos: 1) Teste de concentração inibitória mínima (CIM) para estabelecer a concentração do Farnesol a ser utilizada no estudo, 2) TFDa em culturas planctônicas com ou sem tratamento prévio com Farnesol, 3) Teste de permeabilidade da membrana celular de A. baumannii e análise por microscopia confocal para avaliar a internalização dos fotossensibilizadores, 4) TFDa em biofilmes para determinar a contagem de UFC/mL de A. baumannii e a viabilidade celular por fluorescência, 5) Ensaios in vivo para avaliar os efeitos da TFDa sobre lesões de queimadura infectadas por A. baumannii em Galleria mellonella. Como resultados, observou-se no teste de CIM que o Farnesol não teve efeito antimicrobiano sobre A. baumannii. A TFDa com AM e FTC reduziu, respectivamente, 4 e 2 log (UFC/mL) de A. baumannii em crescimento planctônico. Ambos os fotossensibilizadores foram capazes de penetrar nas células bacterianas, porém o Farnesol não aumentou a permeabilidade celular. A TFDa com AM reduziu 4 log (UFC/mL) de A. baumannii em biofilmes, já a TFDa com FTC não apresentou efeito sobre a contagem de UFC/mL. A aplicação da TFDa com FTC não teve efeito sobre as lesões de queimadura em G. mellonella, entretanto a TFDa com AM aumentou a sobrevida das larvas em 35%. Concluiu-se que a TFDa com FTC teve ação antimicrobiana apenas em culturas planctônicas de A. baumannii e a ação do Farnesol associada a TFDa foi cepa dependente. Dentro dos parâmetros testados nesse estudo, a atividade antimicrobiana da TFDa com FTC foi inferior ao AM nos ensaios in vitro e in vivo


Antimicrobial photodynamic therapy (aPDT) has been an alternative for the treatment of skin infections caused by Acinetobacter baumannii. However, it is necessary to search for photosensitizers or compounds that enhance their effects. The objectives of this study were to test a new Chlorine e-6 (Fotoenticine FTC) as a photosensitizer for aPDT on A. baumannii, and Farnesol as an enhancer for this therapy. In addition the effects of FTC were compared to Methylene Blue (MB), a photosensitizer already approved for clinical use. For this, the following methods were used: 1) Minimum inhibitory concentration test (MIC) to establish the concentration of Farnesol to be used in the study, 2) aPDT in planktonic cultures with or without previous treatment with Farnesol, 3) Permeability test of the cell membrane of A. baumannii and analysis by confocal microscopy to assess the internalization of photosensitizers, 4) aPDT in biofilms to determine the CFU/mL count of A. baumannii and cell viability by fluorescence, 5) In vivo assays to evaluate the effects of aPDT on burn injuries infected by A. baumannii in Galleria mellonella. As a result, it was observed in the MIC test that Farnesol had no antimicrobial effect on A. baumannii. aPDT with MB and FTC reduced, respectively, 4 and 2 log (CFU/mL) of A. baumannii in planktonic growth. Both photosensitizers were able to penetrate bacterial cells, however Farnesol did not increase cell permeability. aPDT with MB reduced 4 log (CFU/mL) of A. baumannii in biofilms, whereas aPDT with FTC had no effect on the CFU/mL count. The application of aPDT with FTC had no effect on burn injuries in G. mellonella, however aPDT with MB increased the larvae survival by 35%. It was concluded that aPDT with FTC had antimicrobial action only in planktonic cultures of A. baumannii and the action of Farnesol associated with PDT was a dependent strain. Within the parameters tested in this study, the antimicrobial activity of aPDT with FTC was lower than MB in in vitro and in vivo assays

ABSTRACT

A terapia fotodinâmica (TFD) vem se mostrando como um método eficaz no controle de micro-organismos patogênicos, sendo investigada para o tratamento de diversas doenças infecciosas, como a candidose bucal. Recentemente, alguns agentes potencializadores dessa terapia têm sido estudados, incluindo a Quitosana (QT), um polímero natural extraído do exoesqueleto de crustáceos. O objetivo desse estudo foi avaliar o efeito potencializador da QT na TFD mediada pelo fotossensibilizador Azul de Metileno (AM) sobre cepas de Candida albicans, investigando sua ação em culturas planctônicas, biofilmes e células persistentes ao fluconazol. Além disso, foi avaliado a capacidade da QT em interferir na absorção do AM pelas células de Candida. Foram utilizadas duas cepas de C. albicans, sendo uma padrão (ATCC 18804) e uma clínica isolada de candidose orofaríngea (70). Para os ensaios em culturas planctônicas, as cepas de C. albicans foram cultivadas em caldo Yeast Nitrogen Base (YNB) por 24 horas. Os biofilmes foram formados por 48 horas no fundo das placas de 96 poços. Para indução de células persistentes, C. albicans foi cultivada com altas concentrações de fluconazol por 48 horas. A seguir, foram realizados os tratamentos com QT a 5 mg/mL (pH de 6,5), AM nas concentrações de 300 ou 600 µM, e irradiação com LED (660 nm) na densidade de energia de 30 J/cm2. Foram incluídos oito grupos experimentais: TFD com AM e QT na presença de Luz (AM+QT+L+), AM e QT sem Luz (AM+QT+L-), QT e Luz (QT+L+), QT sem Luz (QT+L-), TFD com AM e Luz (AM+L+), AM sem Luz (AM+L-), Solução Fisiológica com Luz (F-L+) e apenas Solução Fisiológica (F-L-). Após os tratamentos, foi realizada contagem de unidades formadoras de colônias (UFC/mL). Para determinar a absorção do fotossensibilizador pelas células de C. albicans, as células foram lisadas e centrifugadas para leitura da densidade óptica. Os dados foram analisados por ANOVA e teste de Tukey (p<0,05%). Na TFD em culturas planctônicas, o AM (300 µm) reduziu as células de Candida em 1,6 log (UFC/mL), enquanto que a associação AM+QT levou à redução de 4,7 log. Na TFD em biofilmes, ocorreu redução microbiana de 2,9 log para o tratamento com AM (600 µm) e de 5,3 log para AM+QT. Em relação às células persistentes, a redução encontrada foi de 0,8 log para AM e 1,5 log para AM+QT. No teste de absorção, a penetração do AM nas células de Candida (DO 0,02) foi aumentada na presença da QT (DO 0,39). Concluiu-se que a QT potencializou o efeito antimicrobiano da TFD em culturas planctônicas, biofilmes e células persistentes de C. albicans, provavelmente por facilitar a penetração do AM nas células fúngicas.


Photodynamic therapy (PDT) has been shown to be an effective method to control pathogenic microorganisms, being investigated for the treatment of several infectious diseases, such as oral candidiasis. Recently, some agents that enhance this therapy have been studied, including Chitosan (CS), a natural polymer extracted from the exoskeleton of crustaceans. The aim of this study was to evaluate the potentiating effect of CS on Methylene Blue (MB) photosensitizer-mediated PDT on Candida albicans strains, investigating its action on planktonic cultures, biofilms and persister cells to fluconazole. In addition, the ability of CS to interfere with MB absorption by Candida cells was evaluated. Two strains of C. albicans were used, one standard (ATCC 18804) and one isolated clinical of oropharyngeal candidosis (70). For assays in planktonic cultures, C. albicans strains were cultivated in Yeast Nitrogen Base broth (YNB) for 24 hours. Biofilms were formed for 48 hours at the bottom of 96-well plates. For the induction of persister cells, C. albicans was cultivated with high concentrations of fluconazole for 48 hours. Next, treatments were performed with CS at 5 mg/mL (pH 6.5), MB at concentrations of 300 or 600 µM, and irradiation with LED (660 nm) at an energy density of 30 J/cm2. eight experimental groups were included: PDT with MB and CS in the presence of Light (MB+CS+L+), MB and CS without Light (MB+CS+L-), CS and Light (CS+L+), CS without Light (CS +L-), PDT with MB and Light (MB+L+), MB without Light (MB+L-), Physiological Solution with Light (F-L+) and Physiological Solution only (FL-). After the treatments, colony forming units (CFU/mL) were counted. To determine the absorption of the photosensitizer by C. albicans cells, the cells were lysed and centrifuged for optical density reading. Data were analyzed by ANOVA and Tukey test (p<0.05%). In PDT in planktonic cultures, MB (300 µm) reduced Candida cells by 1.6 log (CFU/mL), while the MB+CS association led to a 4.7 log reduction. In PDT in biofilms, there was a microbial reduction of 2.9 log for the treatment with MB (600 µm) and of 5.3 log for MB+CS. Regarding persister cells, the reduction found was 0.8 log for MB and 1.5 log for MB+CS. In the absorption test, the penetration of MB into Candida cells (OD 0.02) was increased in the presence of CS (OD 0.39). It was concluded that CS potentiated the antimicrobial effect of PDT in planktonic cultures, biofilms and persister cells of C. albicans, probably by facilitating the penetration of MB into fungal cells .

ABSTRACT Objective: The objective was to evaluate the efficacy of gabapentin in the management of neuropathic pain in patients with keratoconus, who were treated with fast (10 minutes) epi-off corneal crosslinking (CXL). Methods: This was a prospective, double-blind, randomized study. The sample comprised patients with bilateral progressive keratoconus, aged 12 years or older, who underwent a bilateral epi-off corneal CXL (fast - 10 minutes) procedure. One group was given placebo orally, and the other group received gabapentin 600 mg orally, both preoperatively. The visual analogue scale (VAS) was applied to record postoperative pain up to 48 hours after procedure. The study was conducted at the Belotto Stock Centro Oftalmológico, in the city of Joaçaba, Santa Catarina, Brazil, from June 2018 to September 2019. Results: At no point in the study significant differences were observed between groups, in terms of pain intensity measured by means of the VAS questionnaire, or of opioid use (Paco®), though opioid consumption was 21% lower in the group receiving gabapentin. Conclusion: We concluded gabapentin has no efficacy in postoperative pain control after epi-off corneal CXL (fast - 10 minutes). Although there was no statistically significant difference, the group that received gabapentin suffered less pain, resulting in lower opioid consumption. UTN number: U1111-1256-0330.


RESUMO Objetivo: Avaliar a eficácia do uso da gabapentina no manejo da dor neuropática em pacientes portadores de ceratocone submetidos ao tratamento de crosslinking corneano epi-off fast de 10 minutos. Métodos: Tratou-se de pesquisa prospectiva, duplo-cega, randomizada. A amostra foi composta de pacientes com ceratocone progressivo bilateral, a partir dos 12 anos de idade, submetidos ao procedimento de crosslinking corneano acelerado epi-off fast de 10 minutos bilateral. Um grupo recebeu placebo via oral e o outro, gabapentina 600mg, via oral, ambos no pré-operatório. A Escala Visual Analógica foi aplicada para registrar a dor pós-operatória até 48 horas após o procedimento. A pesquisa foi realizada no período de junho de 2018 a setembro de 2019 em um centro oftalmológico. Resultados: Não foram observadas diferenças estatísticas significativas para ambos os grupos, tanto na intensidade da dor medida pela Escala Visual Analógica, como na redução do uso do opioide (Paco®), em qualquer horário analisado durante um período de 48 horas. No entanto, houve redução de 21% no consumo de opioides pelo grupo que fez uso da gabapentina. Conclusão: A gabapentina não demonstrou eficácia no controle da dor no pós-operatório do crosslinking corneano epi-off fast de 10 minutos. No entanto, observou-se que, mesmo não havendo diferença estatisticamente significativa, houve diminuição da dor no grupo em que foi usada a gabapentina, resultando na redução do consumo de opioides. PDT has the advantages of minimally invasive, low toxicity, high selectivity, good reproducibility, protection of appearance and vital organ function, and has become a treatment. With the development of medicine, the field of application of PDT becomes more wildly, and brings a new direction for the treatment of oral diseases. This article reviews the basic principles, treatment elements and research results of PDT in the treatment of oral diseases.

ABSTRACT

Photoimmunotherapy (PIT) is an emerging tumor-targeted phototherapy that combines the tumor specificity of monoclonal antibodies with the phototoxicity of light absorbers to rapidly and selectively induce the immunogenic death of target tumor cells. PIT has minimal side effects due to its high specificity. The immunogenic cell death induced by PIT results in rapid maturation of immature dendritic cells proximal to dying tumor cells. Subsequently, the mature dendritic cells present the tumor antigens to CD8+ T cells and induce their activation and proliferation, thus enhancing the antitumor immune response of the host. PIT can also improve the anticancer efficacy by enhancing the penetration of nanomedicines into tumor tissues. In view of the excellent application prospects of PIT, this review summarizes the advances in the immune activation mechanism of PIT, the superenhanced permeability and retention effect, and the new strategies for combinatory therapy, providing references for future research and clinical translation.

ABSTRACT Purpose: This study was performed to evaluate the outcomes of accelerated corneal cross-linking in keratoconic corneas with thinnest pachymetry values of <400 µm. Methods: The study included 28 eyes of 24 patients. The uncorrected and best-corrected visual acuities (logMAR), flattest and steepest keratometric readings, central corneal thickness at the thinnest point, corneal higher-order aberrations, and contrast sensitivity were assessed before and at 1, 3, 6, 12, and 24 months after corneal cross-linking. Result: The mean best-corrected visual acuity and contrast sensitivity increased (p=0.02, p=0.03, respectively), whereas the mean uncorrected visual acuity did not significantly differ (p>0.05) at 24 months after corneal cross-linking, compared with measurements before corneal cross-linking. Although the mean flattest keratometric reading showed no significant change (p=0.58), the mean steepest keratometric reading was reduced when compared with its value before corneal cross-linking (p=0.001). No change was observed in the mean central corneal thickness at the thinnest point at 24 months after corneal cross-linking, compared with its value before corneal cross-linking (p=0.12). Conclusion: Accelerated corneal cross-linking in keratoconic eyes with thin corneas could halt the progression of keratoconus in corneas thinner than 400 µm at 24 months after treatment.


RESUMO Objetivo: Este estudo foi realizado para avaliar os resultados do cross-linking corneano acelerado em córneas ceratocônicas com os valores mais baixos de paquimetria <400 µm. Métodos: O estudo incluiu 28 olhos de 24 pacientes. As acuidades visuais não corrigidas e melhor corrigidas (logMAR), leituras ceratométricas mais planas e íngremes, espessura corneana central no ponto mais fino, aberrações corneanas de mais alta ordem e a sensibilidade ao contraste foram avaliadas antes e em 1, 3, 6, 12 e 24 meses após a realização do do cross-linking. Resultados: A média da acuidade visual melhor corrigida e a sensibilidade ao contraste aumentaram (p=0,02, p=0,03, respectivamente), enquanto a média da acuidade visual não corrigida não diferiu significativamente (p>0,05) aos 24 meses após o cross-linking, comparada com medidas antes do procedimento. Embora a leitura da média da ceratometria mais plana não tenha apresentado alteração significativa (p=0,58), a leitura ceratométrica mais íngreme diminuiu quando comparada ao seu valor antes do cross-linking (p=0,001). Não foi observada alteração na média da espessura corneana central no ponto mais fino aos 24 meses após o cross-linking em comparação com seu valor antes do procedimento (p=0,12). Conclusão: O cross-linking corneano acelerado nos olhos ceratocônicos com córneas finas pode interromper a progressão do ceratocone nas córneas mais finas que 400 µm 24 meses após o tratamento.

RESUMEN Objetivos: Evaluar la actividad fotodinámica in vitro de la ftalocianina de aluminio tetrasulfonada clorada (AlPcClS4) sobre promastigotes y amastigotes de Leishmania (Viannia) peruviana y Leishmania (Viannia) braziliensis. Materiales y métodos: La actividad del tratamiento fotodinámico empleando AlPcClS4 sobre promastigotes y amastigotes de Leishmania fue determinada mediante el método colorimétrico Metil Tiazol Tetrazolium (MTT) y PCR cuantitativo, respectivamente. Resultados: El tratamiento fotodinámico presentó un efecto inhibitorio sobre promastigotes, principalmente sobre Leishmania (V.) peruviana, en menor proporción sobre Leishmania (V.) braziliensis y sobre las formas intracelulares de ambas especies. En Leishmania (V.) peruviana, a las 24 horas posirradiación a 200 µM y 350 µM el efecto inhibitorio fue del 72,9% y 73,9%, respectivamente y a las 96 horas fue del 78,8% y 80,6%, respectivamente. En las formas intracelulares, empleando 200 µM y evaluado a las 72 horas postratamiento, se observó una inhibición del 57,8% de amastigotes de Leishmania (V.) peruviana. El IC50 fue del 56,5; 50; 44; y 39,7 µM, que corresponde a las 24, 48, 72 y 96 horas posirradiación, respectivamente. Conclusiones: El tratamiento fotodinámico empleando AlPcClS4 frente a las especies de Leishmania presentó resultados alentadores principalmente sobre Leishmania (V.) peruviana, lo cual sugiere su potencial uso como alternativa o complemento del tratamiento convencional de la leishmaniasis tegumentaria. Sin embargo, aún se requiere continuar con nuevos ensayos para determinar el índice de selectividad sobre el parásito en su forma intracelular, y desarrollar estrategias que faciliten el ingreso eficiente de la molécula hacia la célula hospedera y al parásito.


ABSTRACT Objectives: To evaluate the in vitro photodynamic activity of aluminum phthalocyanine tetrasulfonate chloride (AlPcClS4) on promastigotes and amastigotes of Leishmania (Viannia) peruviana and Leishmania (Viannia) braziliensis. Materials and methods: The activity of photodynamic therapy using AlPcClS4 on Leishmania promastigote and amastigotes was determined by the Methyl Thiazole Tetrazolium (MTT) colorimetric method and quantitative PCR, respectively. Regarding intracellular forms, the inhibitory effect on Leishmania (V.) peruviana amastigotes was 57.8% at 72 hours post-treatment, using a concentration of 200 μM. The IC50 was 56.5, 50, 44 and 39.7 μM, at 24, 48, 72 and 96 hours post-radiation, respectively. Conclusions: Photodynamic therapy using AlPcClS4 against Leishmania species showed encouraging results, mainly on Leishmania (V.) peruviana, suggesting a potential use as an alternative or complement to the usual treatment of tegumentary leishmaniasis. However, new trials are still required to determine the selectivity index for the intracellular form of the parasite, and to develop methods to facilitate the efficient entry of the molecule into the host cell and the parasite.

RESUMEN: La caries dental no tratada es la enfermedad crónica más frecuente en adultos y la caries no tratada en dientes temporales es la décima enfermedad crónica más frecuente. En Chile los adultos presentan un COPD promedio entre 3,76 - 22,16, pero los estudios no son en población representativa y no existen estudios comparativos entre ceod/ COPD e ICDASTM. A nivel mundial, su prevalencia puede alcanzar el 100 % de la población estudiada. Se buscará evaluar si existen diferencias en la sensibilidad y especificidad en la detección de caries utilizando ICDASTM y ceod/COPD. Se realizó una revisión sistemática cualitativa realizando una búsqueda sobre los índices de caries ceod/COPD y sistema ICDASTM, tanto en Chile como a nivel mundial, en las bases de datos Medline, ClinicalKey y SciELO encontrando 3.581, de ellos, 21 se leyeron por completo y 14 cumplieron nuestros criterios. La sensibilidad y especificidad es mayor con ICDASTM que con el índice ceod/COPD y entrega hasta un 43 % más información al detectar lesiones no cavitadas, pero necesita de mayor tiempo para su realización y más recursos al utilizar luz, aire comprimido y una profilaxis previa al examen. Capacitaciones previas, conocimientos y experiencia en ICDASTM son fundamentales para el mejor resultado, estudios demostraron que mientras mayor experiencia, la sensibilidad y especificidad aumentaba a diferencia del índice ceod/COPD. Se deben realizar más estudios comparativos entre ambos índices e incluir ICDASTM en niños por su capacidad de detectar lesiones no cavitadas las cuales son las más prevalentes en dientes temporales.


ABSTRACT: Untreated dental caries is the most frequent chronic disease in adults and untreated caries in primary teeth is the tenth most frequent chronic disease. In Chile, adults have an average DMFT between 3.76-22.16, but the studies are not in a representative population and there are no comparative studies between dmft/DMFT and ICDASTM. Worldwide, its prevalence can reach 100 % of the population studied. The objective of the study was to evaluate if there are differences in the sensitivity and specificity in the detection of caries using ICDASTM and dmft/DMFT. A qualitative systematic review was done, carrying out a search on the dmft/DMFT index and the ICDASTM system, both in Chile and worldwide, in the Medline, ClinicalKey and SciELO databases, finding 3,581, of which, 21 were read completely and 14 met the inclusion and exclusion criteria. The sensitivity and specificity is greater with ICDASTM than with the dmft/DMFT index and delivers up to 43 % more information when detecting non-cavitated lesions, but it needs more time for its realization and more resources because it needs to use light, compressed air and prophylaxis before the examination. Previous training, knowledge and experience in ICDASTM are fundamental for the best results. Studies showed that the greater the experience, the sensitivity and specificity increased, unlike the dmft/DMFT index. More comparative studies should be conducted between both indexes and include ICDASTM in children for their ability to detect non-cavitated lesions, which are the most prevalent in temporary teeth.

ABSTRACT

Photodynamic therapy (PDT) promotes cell death, and it has been successfully employed as a treatment resource for neuropathic complications of diabetes mellitus (T1DM) and hepatocellular carcinoma. The liver is the major organ involved in the regulation of energy homeostasis, and in pathological conditions such as T1DM, changes in liver metabolic pathways result in hyperglycemia, which is associated with multiple organic dysfunctions. In this context, it has been suggested that chlorophyll-a and its derivatives have anti-diabetic actions, such as reducing hyperglycemia, hyperinsulinemia, and hypertriglyceridemia, but these effects have not yet been proven. Thus, the biological action of PDT with chlorophyll-a on hepatic parameters related to energy metabolism and oxidative stress in T1DM Wistar rats was investigated. Evaluation of the acute effects of this pigment was performed by incubation of isolated hepatocytes with chlorophyll-a and the chronic effects were evaluated by oral treatment with chlorophyll-based extract, with post-analysis of the intact liver by in situ perfusion. In both experimental protocols, chlorophyll-a decreased hepatic glucose release and glycogenolysis rate and stimulated the glycolytic pathway in DM/PDT. In addition, there was a reduction in hepatic oxidative stress, noticeable by decreased lipoperoxidation, reactive oxygen species, and carbonylated proteins in livers of chlorophyll-treated T1DM rats. These are indicators of the potential capacity of chlorophyll-a in improving the status of the diabetic liver.

ABSTRACT

O tratamento endodôntico de dentes permanentes jovens com infecções pulpares/periapicais antes de completar a rizogênese ainda é um desafio para a Endodontia e a Odontopediatria. Relatos científicos têm mostrado que a curcumina (CUR), um fitoquímico polifenólico, apresenta diversas propriedades terapêuticas, entre as quais, amplo espectro de ação antimicrobiana e a capacidade de induzir a proliferação e migração celular. Além disso, devido à sua capacidade excitatória na presença de luz, a CUR também tem sido utilizada como fotossensibilizante em terapia fotodinâmica associada ao LED (light emitting diode), promovendo aumento dos seus efeitos biológicos. Uma forma de aumentar seu potencial terapêutico e reduzir algumas limitações do uso da CUR é a síntese de análogos a partir de pequenas modificações químicas na estrutura original, entretanto, mantendo sua capacidade fotossensibilizante. O objetivo desse estudo foi avaliar a ação antimicrobiana e antibiofilme de análogos de curcumina sob a influência ou não do LED sobre microrganismos de interesse endodôntico e sua influência sobre a viabilidade, proliferação e migração de fibroblastos da linhagem L-929. Uma série de compostos análogos de CUR (PCR-4 H, PCR-3 OH, PCR-4 OH, PCR-3 OCH3, PCR-4 OCH3, PCR-3 acetil, PCR-4 acetil) foram sintetizados pela metodologia de Pabon. A atividade antimicrobiana da CUR e seus análogos foi determinada pelo ensaio de Concentração Inibitória Mínima (CIM) e Concentração Bactericida Mínima (CBM) sobre Streptococcus mutans, Lactobacillus casei, Actinomyces israelii, Enterococcus faecalis e Fusobacterium nucleatum, sob a ação ou não do LED InGaN (nitreto de gálio e índio, com potência de saída de 100 mW/cm², ponta do LED com área de 0,78 cm², 60 s). A curcumina e seu análogo com melhor efeito antimicrobiano (PCR-3 OH) foi avaliado sobre o biofilme inicial (72h) e maduro (1 semana) dessas espécies em microplacas e sobre biofilmes multiespécies formados em túbulos dentinários por contagem das UFC/mL e por microscopia confocal, respectivamente, sob ação ou não do LED. Também foram avaliados quanto à citotoxicidade e a capacidade de induzir proliferação e migração em fibroblastos, por meio de ensaios de metiltetrazólio, azul de tripan e azul de Coomassie, respectivamente. Os dados foram avaliados estatisticamente (p<0,05). Dos 7 análogos de curcumina sintetizados, PCR-3 OH foi o único composto que apresentou atividade bactericida quando testado sobre as bactérias de interesse endodôntico selecionadas. Seu efeito foi potencializado na presença do LED, variando entre as espécies bacterianas. A curcumina teve efeito bactericida para as espécies S. mutans, A. israelii, L. casei e F. nucleatum, e em algumas delas, foi independente do LED. Ambos os compostos reduziram o crescimento dos biofilmes iniciais ou maduros, independente do LED. Entretanto, quando irradiados, o efeito dos compostos variou de acordo com a espécie bacteriana, sendo que A. israelii e S. mutans foram os mais afetados. Ambos os compostos reduziram significativamente os biofilmes multiespécies quando comparados ao controle sem tratamento, sendo que melhor efeito foi observado para PCR-3 OH. A curcumina foi considerada citocompatível a partir de 0,039µg/mL e PCR-3 OH a partir de 0,019 µg/mL. Houve redução significativa na viabilidade celular quando os compostos foram irradiados com LED nas concentrações 0,039 e 0,019 µg/mL. O LED, dentro dos parâmetros testados, reduziu significativamente a viabilidade, a proliferação e a migração celular, independente do composto ou tempo de exposição. Conclui-se que PCR-3 OH apresentou atividade bactericida e sobre biofilmes simples e multiespécies de bactérias de interesse endodôntico superior à CUR, principalmente sob ação do LED. Entretanto, sua citocompatiblidade foi inferior à da CUR. A presença do LED afetou a viabilidade, proliferação e migração dos fibroblastos, mostrando que os parâmetros utilizados para fins antimicrobianos não foram adequados para aplicação em células eucarióticas(AU)


Endodontic treatment of young permanent teeth with pulp / periapical infections before completing rhizogenesis is still a challenge for Endodontics and Pediatric Dentistry. Scientific reports have shown that curcumin (CUR), a polyphenolic phytochemical, has several therapeutic properties, including a broad spectrum of antimicrobial action and the ability to induce cell proliferation and migration. In addition, due to its excitatory capacity in the presence of light, CUR has also been used as a photosensitizer in photodynamic therapy associated with LED (light emitting diode), promoting an increase in its biological effects. One way to increase its therapeutic potential and reduce some limitations of the use of CUR is the synthesis of analogues from small chemical modifications in the original structure, however, maintaining its photosensitizing capacity. The aim of this study was to evaluate the antimicrobial and antibiofilm action of curcumin analogues under the influence or not of LED on microorganisms of endodontic interest and their influence on the viability, proliferation and migration of L-929 fibroblasts. A series of CUR analog compounds (PCR-4 H, PCR-3 OH, PCR-4 OH, PCR-3 OCH3, PCR-4 OCH3, PCR-3 acetyl, PCR-4 acetyl) were synthesized by Pabon's methodology. The antimicrobial activity of CUR and its analogs was determined by the Minimum Concentration Inhibitory (CIM) and Minimum Bactericidal Concentration (CBM) assay on Streptococcus mutans, Lactobacillus casei, Actinomyces israelii, Enterococcus faecalis and Fusobacterium nucleatum, with or without the InGaN LED (gallium and indium nitride, with output power of 100 mW / cm², LED tip with an area of 0.78 cm², 60 sec). Curcumin and its analog with the best antimicrobial effect (PCR-3 OH) were evaluated on the initial (72h) and mature (1 week) biofilm of these species in microplates and on multispecies biofilms formed in dentinal tubules by counting CFU / mL and by confocal microscopy, respectively, under the action or not of the LED. They were also evaluated for cytotoxicity and the ability to induce proliferation and migration in fibroblasts, using methyltetrazolium, trypan blue and Coomassie blue assays, respectively. The data were evaluated statistically (p <0.05). Of the 7 curcumin analogues synthesized, PCR-3 OH was the only compound that showed bactericidal activity when tested on selected bacteria of endodontic interest. Its effect was enhanced in the presence of LED, varying between bacterial species. Curcumin had a bactericidal effect for the species S. mutans, A. israelii, L. casei and F. nucleatum, and in some of them, it was independent of the LED. Both compounds reduced the growth of the initial or mature biofilms, regardless of the LED. However, when irradiated, the effect of the compounds varied according to the bacterial species, with A. israelii and S. mutans being the most affected. Both compounds significantly reduced multispecies biofilms when compared to the untreated control, with the best effect being observed for PCR-3 OH. Curcumin was considered cytocompatible from 0.039 µg / mL and PCR-3 OH from 0.019 µg / mL. There was a significant reduction in cell viability when the compounds were irradiated with LED at concentrations of 0.039 and 0.019 µg / mL. The LED, within the parameters tested, significantly reduced cell viability, proliferation and migration, regardless of the compound or time of exposure. It is concluded that PCR-3 OH showed bactericidal activity and on simple and multispecies biofilms of bacteria of endodontic interest superior to CUR, mainly under the action of LED. However, its cytocompatibility was lower than that of the CUR. The presence of the LED affected the viability, proliferation and migration of fibroblasts, showing that the parameters used for antimicrobial purposes were not suitable for application in eukariotic cells(AU)

Introdução: A terapia fotodinâmica (TFD) é utilizada como coadjuvante ao tratamento endodôntico para potencializar a redução microbiana no sistema de canais radiculares. No entanto, a literatura carece de estudos sobre as consequências da TFD na resistência de união do cimento obturador à dentina intrarradicular. O objetivo deste estudo foi avaliar a influência da TFD com azul de metileno na resistência de união do cimento obturador (MTA Fillapex) utilizando o teste de push-out, analisar o padrão de fratura das amostras representativas por microscopia eletrônica de varredura (MEV) e a morfologia da interface adesiva intrarradicular pela microscopia confocal de fluorescência (MCF). Material e Métodos: Cinquenta e cinco dentes bovinos foram utilizados para simular o tratamento endodôntico. O preparo biomecânico foi realizado em todos os canais radiculares e os espécimes foram distribuídos em 5 grupos: controle (água deionizada); azul de metileno 50 mg/L (AM50SL); azul de metileno 100 mg/L (AM100SL); azul de metileno 50 mg/L + laser vermelho 660nm (AM50L) (TFD); e de azul de metileno 100 mg/L + laser vermelho 660nm (AM100L) (TFD). A resistência de união do cimento obturador à dentina foi mensurada usando uma máquina de teste universal (n=8) e as amostra representativas analisadas em MEV. A morfologia da interface adesiva intrarradicular foi analisada pela MCF. Os dados de resistência de união foram submetidos ao teste de KruskalWallis para comparação entre grupos e ao teste de Friedman para comparação entre terços (α=.05). Resultados: Houve diferença estatística significante para o grupo azul de metileno 100 mg/L + laser vermelho 660nm (TFD) quando comparando os terços radiculares, onde o terço apical apresentou maiores valores de resistência de união em relação ao terço médio (P=.0302). Nos demais grupos e terços não houve diferença estatística significante (P>.05). Quanto a morfologia da interface adesiva foi possível observar uma menor penetração do cimento obturador no grupo AM100L. Conclusão: Pode-se concluir que o uso de TFD com o fotossensibilizador azul de metileno na concentração de 50 mg/L não alterou negativamente a resistência de união do cimento obturador MTA Fillapex na dentina intrarradicular(AU)


Introduction: Photodynamic therapy (PDT) is used as an adjunct to endodontic treatment to enhance microbial reduction in the root canal system. However, studies are lacking on the consequences of the application of the application of PDT on the bond strength of the root canal sealer. Therefore, the objective of this study was to evaluate the influence of photodynamic therapy with methylene blue on the bond strength the root canal sealer using a push-out test, to analyze the fracture pattern of representative samples using scanning electron microscopy (SEM) and the morphology of the adhesive interface of the intraradicular dentin using Confocal Laser Scanning Microscopy (CLSM). Material and Methods: Fifty-five bovine teeth were used to simulate experimental endodontic treatments. Biomechanical instrumentation was performed for all root canals, and the teeth were distributed into 5 groups: control deionized water; methylene blue 50 mg/L (MB50WL); methylene blue 100 mg/L (MB100WL); methylene blue 50 mg/L + red laser 660nm (MB50L) (PDT); and methylene blue 100 mg/L + red laser 660nm (MB100L) (PDT). The push-out bond strength of the root sealer to dentin was measured using a universal testing machine (n=8), representative scanning electron microscopy images were obtained. Images of the morphology of the adhesive interface were obtained using CLSM (n=3). Bond strength data were submitted to the non-parametric Kruskal-Wallis test for comparison between groups and the Friedman test for comparison between thirds (α=.05). Results: Comparing the root thirds, for the methylene blue group with the higher concentration activated by Red Laser, there was significant difference, the apical third presented higher values of bond strength in relation to the middle third (P=.0302). In the other groups and thirds there was no statistically significant difference (P>.05). As for the morphology of the adhesive interface, it was possible to observe a lower penetration of the sealer in the MB100L group. Conclusion: The application of PDT with the photosensitizer methylene blue at a concentration of 50 mg/L, regardless of the activation by red laser, did not alter negatively the bond strength of the MTA Fillapex sealer to intraradicular dentin(AU)

ABSTRACT

Objective: The aim of this in vitro study was to evaluate the efficacy of photodynamic inactivation (PDI) with erythrosine (E), using a light-emitting diode (LED) on planktonic cultures of Streptococcus mutans. Material and Methods: A Streptococcus mutans strain (UA 159) was used to prepare the suspensions containing 107 cells/mL, which was tested under different experimental conditions: a) LED irradiation in the presence of erythrosine as a photosensitizer (E+L+); b) LED irradiation only (P-L+); c) treatment with erythrosine only (E+L-); and d) no LED irradiation or photosensitizer (P) treatment, which served as a control group (P-L-). After treatment, strains were seeded onto MSBS agar for determination of the number of colony-forming units (CFU/mL). Results: The results were submitted to analysis of variance and the Tukey test (p < 0.05). No reduction in the number of CFU/mL was observed in the treatment group with erythrosine (E+L+) when compared to the control (P-L-). Conclusion: PDI using erythrosine did not reduce the number of CFUs per millimeter within the parameters in this study. (AU)


Objetivo: o objetivo deste estudo in vitro foi avaliar a eficácia da inativação fotodinâmica (PDI) com a eritrosina (E), usando diodo de emissão de luz azul (LED) em culturas planctônicas de Streptococcus mutans. Material e métodos: a cepa de Streptococcus mutans (UA 159) foi usada para o preparo das suspensões padrões contendo 107 células/mL, as quais foram testadas em diferentes condições experimentais a) irradiação com LED em presença da eritrosina como fotossensibilizador (E+L+); b) irradiação com LED apenas (F-L+); c) tratamento com eritrosina apenas (E+L-); e d) tratamento sem irradiação com LED ou fotossensibilizador (F), que serviu como grupo controle (F-L-). Após o tratamento, as cepas foram semeadas em ágar MSBS para determinação do número de unidades formadoras de colônias (UFC/mL). Resultados: os resultados foram submetidos à análise de variância e teste de Tukey (p < 0.05). Não foi observada redução no número de UFC/mL no grupo de tratamento com eritrosina (E+L+) quando comparado ao grupo controle (F-L-). Conclusão: a PDI usando etritrosina e LED não reduziu o número de UFCs por milímetro com os parâmetros utilizados neste estudo.(AU)

ABSTRACT

Las fotodermatosis son un conjunto de patologías cutáneas originadas o agravadas por exposición a radiación ya sea solar o artificial. Se clasifican en cuatro categorías: 1. Idiopáticas o mediadas inmunológicamente 2. Dermatosis fotoagravadas, 3. Fotosensibilidad inducida por agentes y 4. Trastornos por reparación defectuosa del ADN. La fotosensibilidad inducida por agentes consiste en reacciones secundarias a la exposición de ciertos químicos, llamados fotosensibilizadores y a distintos tipos de radiación lumínica. Los fotosensibilizadores pueden ser de origen endógeno o exógeno, aquellos exógenos provienen desde el ambiente, fármacos u otros productos (tanto sistémicos como tópicos), los cuales sufren modificaciones estructurales al entrar en contacto con radiación, provocando como consecuencia, distintas manifestaciones cutáneas. En este artículo se revisarán principalmente las reacciones fototóxicas y fotoalérgicas (ambas, reacciones de fotosensibilidad inducidas por agentes exógenos) indagando en sus diferencias y el enfrentamiento clínico de cada una. También, se revisarán los exámenes que permiten estudiar los distintos diagnósticos diferenciales, especialmente el test de fotoparches, el cual está cobrando cada vez más importancia en la práctica clínica.


Photodermatoses are a group of skin diseases induced or aggravated by exposure to radiation, whether solar or artificial. They are classified into four general categories: 1. Idiopathic or immunologically mediated photodermatoses 2. Photoexacerbated dermatoses 3. Agent induced photosensitivity 4. DNA repair defects Photosensitivity induced by agents are secondary reactions to the exposure to some chemicals, called photosensitizers, and to different types of light radiation. Photosensitizers can be classified as exogenous or endogenous. Exogenous agents come from the environment, drugs or other products (both systemic and topical), which undergo structural changes when they come into contact with radiation, causing different skin manifestations as consequence. Differences between phototoxic and photoallergic reactions (both photosensitivity reactions induced by exogenous agents), the clinical approach of each one of them, and available tests that are used to make a diagnosis, especially, photo patch test will be reviewed in this article
